A 67-year-old man is transferred directly from the Emergency Department to theatre with a ruptured abdominal aortic aneurysm. He is drowsy, clammy, and profoundly shocked despite aggressive resuscitation. Blood pressure remains critically low, capillary refill is prolonged, and he is becoming increasingly confused. Bedside imaging confirms a large leaking aneurysm with free intra-abdominal blood. The vascular team advise that without immediate operative intervention, survival is extremely unlikely.
